Good day class especially to my L406 OJT students. In this video, I'm gonna discuss our requirements for this particular um subject. Although we already have been given a heads up on this uh during the syllabus orientation um but this one has specifics in terms of what exactly is expected from each of the assessments which we used to refer to learning evidence. So let's proceed right away to the lists of all four requirements. Um in this uh semester uh it has been called um either uh formative or summative assessment. While formative assessments may not be graded, they are still to be accomplished um just so you have completed all the requirements. So formative assessment 1 or FA1 uh comprises your job application requirements. Formative assessment 2 or FA2 is interview. This one is something we have indicated in the syllabus but as an alternative to this is a prerecorded one minut video of selftroduction instead not anymore interview with your cooperating teacher and you as the applicant for the internship but just a self introduction a video recorded self introduction and one minute at that. The third is a summative assessment one or SA one which is your practicum portfolio and it's going to be 30% of your grade. The second and the fourth sa um fourth assessment is summative assessment 2 or sum or sa 2 and this is employer evaluation which will comprise 70% of your grade. So formative assessment one or your job application requirements um is uh going to include this one. So let me share uh our UV. Okay. So this is how our UV looks like. It has a tab labeled as general and if youck announcements you be brought to the link to announcements and I have only placed in here the link to the letter hand. So these are files shared by the unersity which are all relevant to official letter head. And then next tab is OJT syllabus. You have already access the syllabus in PDF format and have also watched um via YouTube the syllabus orientation. This is just the recording of our first uh meeting ever online meeting at that. And then the third tab would be course overview. It has the list of modules which are job application, ethics and antiual harassment act. For the job application this is going to be a workshop no of the of the requirements of the formative assessments that you will submit. Okay. And then the work ethics and the antiseual harassment app covered during the preinternship orientation in the social hall which we had last March um first week of March no and then course contents uh researches supplementary materials and finally the most important one would be the last tab which is assessments. When you click OJT assessment submission menu will be brought to the link to the X Google sheet. No in this a sheet you have here tab below FA1 and it asks you for the link to your application letter. Uh and also in the same uh tab will be where the grading will be indicated. Okay. And then another tab is formative assessment number two which is a link to the recorded video. Okay. The recorded video that you just mentioned. So um what I want to focus is this FA1 which is application letter. The application letter um will be graded as follows. First would be linguistic precision which is five points. You have to demonstrate sophisticated vocabulary flawless syntax and grammar and demonstrate high level or applied linguistics competence. This is appropriateness of the content. So for uh in the workshop we had before this second semester, I think in the first semester last year, no when I met you um before we ended our uh first semester, you were asked to submit application letters for for the establishments that we targeted. Unfortunately um the MOA took a long time and I don't think it can catch uh it can it can make it on time this time no. So um that's why uh the application letter will have to be uh uh revised to make it appropriate um as an application to a internship. uh position um as uh assistant a teaching assistant here in the college of arts and sciences in the University of South East Philippines. So the high level of competence in applied linguistics here refers to your ability to contextualize your application letter to what's expected um in the job. No what's expected while you are at work. So please um the application letters submitted last year sometime in November and December will have to be revised to be an application to teaching assistant as intern here in the language department for those are um assigned to major classes or uh general education department for those who were only assigned to education and if both um your application will have to highlight language instead no if if you have um ano if you have been assigned to both major classes and gen classes highlight your ability to handle major classes and offer your um ability also to handle general education subjects your application letter must also have structural logic that's five points that's perfect business letter format it says perfect here because we can actually perfect this one no you you review no um how to write a formal letter session are seamless ideas flow logically from book to conclusion third um criterion for grading your application letter will be content and context that's five points what do We mean by five points. This one exp uh your this means that your letter explicitly mentions applied linguistic theories or EFL strategies um and and how will that make you align with our department goals and uh number four or fourth criterion is persuasiveness. This means that you have a strong call to action indicated there. You are able to clearly articulate how you can add value to the department. And finally, five points would be professionalism. Your application letter demonstrates the following neatness, white space balance and professional digital signature. Ideally application letter is only one page. The second form of assessment is your um self introduction video. So earlier when I showed you the link to UV, it also leads to a tab in the Google sheet. uh specifically just for the video it also indicates their criteria for grading and those criteria actually here in this slide the first criterion is linguistic proficiencies or in your video your speech must be clear accurate and perfectly paced for EFL learners and that there are no grammatical errors. The second criterion is that you have to appear professionally. You have to dress in professional business attire with a blazer ideally. No, but if you have um um your uh uh you can wear your practicum uniform type A, not the polo shirt and that your grooming is neat as well. And then the third car is pedagogical presence. You have to project high energy, confidence and warmth and you have to maintain eye contact with the camera. This is just like what I'm doing right now. When I'm not reading the slide, I'm going to look at the camera and imagining that this is the eye of the viewers of this video. And finally, a final criterion is content and organization. This is five points. Your introduction is concise. You follow the logical flow of hook, bridge, and goal and you stay under time. No. So it's it's 1 minute. No one minute sometimes is already really very long no. So you have to train yourself to go directly to the point um and and be able to highlight the important details in a minute. Okay. And finally your technical quality. The background is clean and quiet and that lighting is bright on the face and the audio is crisis. Summative assessment one is your practicum portfolio. It should demonstrate completeness of narrative. That's just that's five points. The narrative is exhaustive and perfectly chronological providing a comprehensive career dier of the entire 300 hourour emission immersion. And then number two, evidence of performance that's five points. You are to provide a rich array of high quality artifacts that serves that serve as a refutable proof of all workplace competencies. So competencies ability to facilitate classes, ability to prepare instructional materials, ability to conduct assessment practices. Those are three major um components of uh teaching position. Okay. And then number three, significant learnings. Your portfolio offers profound transformative reflections that demonstrate deep internalization of workplace growth. And number four real world application which is 5 points. You have masterfully demonstrated theory to practice bridge by applying linguistic expertise to complex problems. So what are the parts of a practicum portfolio? First it must have a cover page. You can follow the thesis title page and you can give your and of course give your own title for practicum experience. After the first page is table of contents then followed by dedication and acknowledgement. Next is your resume or curriculum. Next is your teaching philosophy. This is a 300 to 500 word essay. On your approach teaching adults mention applied linguistic theories like communicative language teaching or taskbased learning and then the script to self introduction video you paste a link here or qr code to video profile of use next after the self introduction video would be the profile of use cast language program education program and the courses that you have handled and next is narrative report 10 highlights include preinternship orientation and send off. Evidence of outputs is the next part. Um, show evidence of IMS of lesson plans if you did allowed to make lesson plans or activities that you have um crafted yourself and then of course the photos with caption appropriate photos with caption. Um so how many photos minimum of three no per output minimum of three per output and then summary insit gain summary expectedly would include insights gained reflection swat analysis of the self swat is strength, weaknesses, opportunities and threats And this one after the summary would be appendices A is your certificate of registration your COR or a certificate of um enrollment. I don't know if you can access the same certificate that I can for my end, but let me show you what I mean by this. So I I hope you can see the sample um certificate of registration and billing here. Uh um this is Erica's ano cor. So as per CMO this is part of your requirement so I don't know if you can access the same sa ano sa inyang portal please confirm with me if not I can I can send you your copies from my end this is something I can access because I am an enrolling officer so I hope that's clear let's go back to our um PowerPoint presentation. So next appendix or appendix B is birth a certificate. Please scan a copy of your PSA birth certificate medical certificate you can just secure this from the barang health center or wherever you can uh get one. You can choose to have this from a private institution. It's up to you. Then your application letter which is address to the dean through the program head. Letter E would be your curriculum v. This can this the length of this is up to you. No then followed by your certificate of completion which is the pre-internship orientation with photos and caption. So one uh one appendix for uh this one no letter G appendix G is certificate of attendance for send of ceremony with photos and caption letter H is internship contract so how does the internship contract look like I'm going to also make this available in the UV so this is our internship contract Okay. In the during the internship uh pre-internship orientation, I was able to mention this one but uh um only now that I I uh although I have shared this to my practic my interns uh but I'm also going to share this to everyone else. So this is the internship contract. Um ah we will just transfer this to the to the appropriate um letter head. No, I have already uh um pasted the link to files um letter head. So what does the internship contract look like? It begins with purpose. It indicates name of parties involved. It says that this contract formalizes the internship arrangement among the University of Southeastern Philippines College of Arts and Sciences and the student intern enrolled in the Bachelor of Arts in English language major and applied physics program. It also enumerates goals and development of specific skills that the internship aims to provide structured and supervis training that applies theoretical knowledge real world context. Develop professional competencies in research, documentation, teaching, editing, communication and applied linguistic tasks and strengthen workplace readiness, ethical conduct and adaptability in professional environments. Brief description of the scope of the agreement. This agreement covers the Oh, this is 300 hourour internship program required of BL students conducted during the second semester of SY 2025 2026. The internship is to be undertaken strictly within Davao City um uh with student internsed only to host training establishments uh within Davao City specifically within specifically in the university of Southeastern Philippines which uh which is or I will add na lang college of arts and sciences. The HDE shall provide supervision mentorship and evaluation of intern performance while umep shall monitor compliance. Okay. So through the um ano through the OJT coordinator that's yours truly shall monitor compliance and ensure academic integration of the internship experience. So there are two key contacts for that will be included here. So for us cast indicate the name of your coordinating professor or supervisor and then um next would be your name and contact number. The internship shall cover 300 hours beginning on January 19, 2026 and ending on May 22, 2026 unless extended or terminated in accordance with this contract. roles and responsibilities. The student interm shall abide by all rules, regulations and codes of conduct of both usef and the perform assigned tasks diligently, safely and ethically following professional decorum. Protect the confidentiality of information acquired during the internship. Attend orientation seminars and training sessions required byf and submit progress reports activity logs and self-evaluations as required. Accept responsibility for personal expenses unless otherwise provided and respect workplace health, safety and quality service standards. Usep shallal endores qualified student interns and ensure submission of required documents such as COR, medical certificate, wavers, if necessary, and many others. conduct preinternship orientation covering institutional policies, professional ethics and standards of quality service which we already did. No monitor student intern progress through reports, faculty visits or consultations require the HD to submit evaluation forms of at least 2 weeks before completion and integrate the internship into the curriculum ensuring that outcomes aligned with program standards and imposed sanctions after due process for student evaluations of this contract the internship plan or the HTE rule. So class you notice item number four is at least 2 weeks before the completion. we will have to ano we will have to um contextualize this no this would refer to submission of uh of the evaluation forms kasi ba supposed to be you may be able to complete the 300 hours on May um 22 but uh you may be able to gather na the evaluation forms um 14 May 8 no first week of May so I'm going to check up on you please update me if you have problems meeting your practicum hours. Um kasi if you would be if you have balance the time it's a max of 6 hours per day no or 4 hours per 4 to 6 hours um per day no so yan um ang DTR pwede pa sa May 22 so we're going to set as well the deadline for the submission of your una of practicong portfolio kasi mas advance no earlier ang submission ng grades ninyo. So hopefully we can have um your portfolio soft copies of them on May 25. Ang hard copy would be May 26. Ganun na lang para ma-submit na ang grades ninyo sa May 27 sa practicum kasi um ano pa ah meron pa tayong mga papers for the thesis. So 2 weeks before May 22 we're going to have to collect the um evaluation forms. You have to secure evaluation forms from your cooperating teacher no May 8. So the as here in the college of arts and sciences we are supposed we are supposed to provide you with meaningful task assign with the Bal internship internship plan ensure close supervision and mentoring by qualified staff in this case we the faculty are helping each other no monitor supervise you secure the safety and welfare of student interns during training hour protect interns from any form of sexual physical or psychological harm consistent with RA7877, RA9262 and RA11313 provide access to relevant facilities, resources and professional experiences. Evaluate student interns fairly, completing required forms and issuing a certificate of completion. notify use promptly for of any major concerns, violations or emergencies involving interns. I um shared to uh Miss Erica the certificate of completion. Um the uh not all of you have the same um major responsibilities. It is you who know what major responsibilities are given to you. So I have shared to Erica that the general ones include preparation of instructional materials, facilitating classes and um conducting assessment activities. But in case you also have other highlights no which you think are important to be included in the certificate of completion please do so. Kaya siya individual na pag-prepare sa certificate of completion. So you guys coordinate na lang with one another especially with your mayor Erica. Um just so you can agree on how the certificates will be submitted. Pwede ng si CAS ang mag-print. Dito na magpa-print. I just need the soft copy. So sa third year si Michael uh Montilloon no um uh you know helps us with uh certificate concerns. So gina-canva niya and then she and he would convert the file into PDF file. Tapos gina-email niya sa akin and then I'll be the one na to have them printed sa dean's office para mapirmahan na rin agad. No, we have two signatories and that's our PH Sajad Ingilan and our dean of course Dr. Marne Grace Somico. So your place of assignment is within Davao City. It's a violation against your rights if we assign you outside no so please inform me if there is such a thing as I don't think there is one like that no. Okay. So that's for your place of assignment compensation or and benefits this is noncensated. I'm sure you're aware of that because this is your um learning opportunity no part of your requirement and confidentiality the student in turn shall maintain strict confidentiality of all information records and materials access during the internship disclosure without prioritate and consent of the HD and is prohibited even beyond the internship period internship placements are valid only within Davao City and under this agreement placement outside Davao City shall not be permitt this contract shall not be construed or to create an employer employee relationship between the HT and the student intern. The student intern shall be liable for any damages to property or injury to third parties caused by intentional or negligent acts committed in the course of training. Effectivity and termination. This contract takes effect upon signing and remains valid until the end of the internship period unless earlier terminated for just caus by any party upon written notice. So signature your signature for the student intern the date class is supposed to be January 12 2026. Coordinating supervisor will have to be the same alamin back in December last year who your advisor would be and your perspective um coordinating professor and then ako sa last as witness no and maybe we can put here 13 instead okay so fresh signature ito class. So um now that we're done with March, you are expectedly ready with your 3DRs. Kailangan siya ng counter sign ng advisor ng cooporating instructor. So start na start na kayo mag-secure ng counter sign no para ma-double check natin how many hours have you already completed and let's see how many more needs to be served in April and May. No so that's practically 1 month and a half na lang no? 1 month and a half. So let's see you have to start counting now and then of course evaluation by cooperating professor transfer the file into the letter headpon uh and follow the CMO but I'm gna I'm gna share na lang din the link to the evaluation form and finally the certificate of completion the duy sign so Erica will have to um follow me up on this kapagka nabigay niya na sa akin yung file it might take a little bit of time to secure the signatures kaya you have to do this ahead of time maybe starting on April 6 maayos na yung certificate para ma-secure na natin ang submission sa din lalo na may mga travels ang ate ang ating mga signatories no so ayusin na natin to and I want uh these requirements to be ready kasi I want you guys to focus on your thesis which is actually more demanding also. Okay. So this is how the um summative assessment number two this is how it looks. Oh class you saw this one na during the orientation no just going to have to repeat this here. Um again the file will be provided to you. So how will you identify department tasks? How will the rating go? Um the supervisors actually know that the rating is our equivalent rating in 1.0 for perfect, 3.0 for passing, 5.0 for failure. No so um here you have to ano you have to indicate tasks and the period. No, the period will have to match the total number of hours. So this is um you start doing this one na class. Hag hwag iano huwag cram ito ha kasi medyo mahaba to at kailangan pa magpa-greed. So pwede ng magpa-greed ha. na magpa-gread. So practically ma-accommodate Saturday ano um apat or anim max of sixuro on a Saturday kasi medyo mabigat na rin if if it's more than that. Usually the final defense is 1.5 hours 1 and 1/2 hours.
