Do you suffer from over-raised whitish lesions that, although they are not pimples, affect the texture of your skin? you could be suffering from closed comedones. In this video I tell you what they are and how you can treat them. acne is a chronic disease of the pilo sebaceous unit. the pathogenesis of acne depends on four fundamental factors: hyper keratinization at the level of the hair follicle, increased sebaceous secretion, the presence of bacteria and the inflammatory response produced by the immune system. To understand acne and the lesions it produces, we must know where it occurs. acne begins here in the pilo sebaceous apparatus which is made up of a follicle, a sebaceous gland and a hair. Around eight or nine years of life prior to puberty, the adrenal gland, a gland that is in close contact with the kidneys, begins to synthesize greater amounts of a hormone called dehydroepiandrosterone sulfate that is transformed several times within the body. once transformed, it is capable of acting on the sebaceous glands of the skin. This stimulation on the sebaceous glands causes them to increase in size which leads to a greater production of sebum. Sebum that, as I said before, is going to be emptied into the light of the follicle. Do you follow me here? It is also known that these hormones stimulate hyper proliferation at the level of the cells that line the hair follicle, that is, the cells that cover the light of the follicle are actively multiplying thanks to the action of these hormones. This increase in cells leads to them accumulating, so what do we have? a hair follicle filled with cells and sebum. As hyperkeratinization progresses and the number of keratinocytes that accumulates at the follicular level increases, lesions called micro comedones appear that precede the formation of open and closed comedones. open eaters are the classic black dots correspond to a dilation of the follicle with a dark almost black surface due to the fact that they contain oxidized melanin. if we press them, which we should not do and I imagine you will not do, we can extract their content. I talked about open dining rooms in another video so you can go watch it after finishing this one. closed dining rooms are those typical injuries with a barely perceptible hole with a whitish surface that may not be seen with the naked eye but that stand out a lot depending on how the light shines on them and these injuries are the protagonists of today's video . If to all this that accumulates and that happens at the level of the hair follicle, we add the presence of a bacteria called Cutynebacterium Acnes and the presence of inflammation caused by a tremendous salad, what do we have? in addition to closed dining rooms and open dining rooms, inflammatory lesions, the typical pustular papules that characterize inflammatory acne. but that is the subject for another video, let's go back to non-inflammatory lesions, to closed dining rooms, if we look at closed dining rooms with magnification we could see a distended follicle that takes on a cystic stress appearance with a thick content made up of keratinocytes, hair bait and bacteria If this lesion continues to fill at a certain point, it ruptures, that content falls into the surrounding dermis, generating inflammation and giving rise to inflammatory lesions. If, on the contrary, these lesions do not break and this material is kept contained within the follicle, we will only suffer from comedonian acne, that is, as long as the closed dining rooms do not break, we will suffer from these annoying lesions that even if we squeeze them, it seems not come out nothing, unlike what happens with blackheads. This happens because the exit orifice of closed comedones is much less dilated than that of open comedones. You probably heard me talk throughout this video and you ran to the bathroom to see if what you had on your forehead were closed comedones, how could you identify if what you are having on your skin are really closed dining rooms? I give you a couple of tips. As I mentioned before, closed comedones are small structures, with a smooth and shiny whitish surface, they usually appear in areas where sebaceous glands abound, such as the forehead, the nasal dorsum, the chin, although they can appear all over the face. open comedones or blackheads may be accompanied by their first cousins and may or may not be accompanied by inflammatory lesions. Can you have closed comedones and not have red bumps or pus? Yeah right . there is a type of acne that is characterized by only presenting comedones. It is usually a very frequent cause of consultation in patients who report having uneven skin texture, this means that you may be suffering from acne for the simple fact of having comedones and you never knew what the risk factors are for suffering from closed comedones ? On the one hand, genetics we can also mention the increase in androgens, an increase in this type of hormones will stimulate the production of sebum and hyper keratinization at the level of the hair follicle, it can be seen at puberty in hormonal therapies, etc. Another risk factor is the use of skin care products or makeup of dubious quality or that are not formulated for use on the face, comedogenic products. We can also mention as a risk factor, humidity, smoking, foods with a high glycemic index, among others, now you know what to suffer from comedones, how do we treat them? Well that's the subject for a next video so if you want a second part of this video about dining rooms, like this video. It is a lie, if you suffer from closed dining rooms, what we should do is the following: sweep the contents of the pore, clean it in depth, normalize the proliferation of cells inside it and destroy the comedo. How do we do it? for this the use of certain active principles and certain products is useful . Among the topical treatments for closed comedones I can mention over-the-counter products and prescription products. First I want to mention benzoyl peroxide, this is an active principle with keratolytic, comedolytic, sebum-regulating and anti-inflammatory properties, everything we need to eliminate comedones. It is used more in cases of inflammatory acne given its ability to reduce inflammation and kill bacteria, it can be used alone or in combination with retinoids or antibiotics. It is not available over the counter in Argentina but has to be prescribed by your dermatologist, but in other Latin American countries and the world you can find over-the-counter products with this active ingredient, such as Cerave's benzoyl peroxide cleanser. then we have azelaic acid, azelaic acid is keratolytic and comedolytic, it destroys comedones, helps to normalize the growth of cells inside the follicle, preventing them from accumulating and also destroying the comedo. In Argentina it can only be obtained by sending it to formulate it, then we have salicylic acid one of my favorite active principles has a keratolytic effect, it helps to regulate the oiliness of the skin, it helps to clean pores in depth since it is lipophilic / acts well with fats It interacts well with sebum and also helps to shed the cells that are inside the hair follicle, preventing them from accumulating and clogging it. We can find salicylic acid in different products such as cleaning products , I can mention them for example We can also find it in products that are applied and left on the skin, such as I can mention glycolic acid as the next active principle . Glycolic acid is used for its exfoliating effect, it is an alpha hydroxide, it makes the cells that make up the most superficial layer of the epidermis detach, we can find it in cleansers such as, for example, we can also find it in gels, creams or serums after mentioning for instance following active principles that I can mention, retinoids, especially adapalene and tretinoin. Retinoids are applied at night to the area we want to treat, it has an antiproliferative effect, modulates growth and epidermal differentiation, promotes normal proliferation in epidermal cells and normalizes epidermal hyper proliferation ... in Spanish They prevent the cells that cover the light of the hair follicle from accumulating and therefore the comedo is formed. They are the first-line treatment for non-inflammatory acne cases in Argentina they are sold under prescription but in some Latin American countries or the world we can find it as an over -the-counter I can mention them for example in case your case warrants it, you can also receive medication by mouth, medication by mouth, it is not the objective of this video to talk about these treatments but know that when acne does not respond to topical treatments or conventional treatments it can be treated with hormonal therapy or isotretinoin. When treating closed comedones with whatever product or active ingredient it is vitally important to use moisturizers and sunscreen because these active ingredients that are used to treat comedones are quite irritating, therefore if we do not hydrate, if not We protect our skin from the sun during this treatment process, the treatment will become very difficult to carry out and we could injure the skin. Everything is very beautiful, everything is very beautiful, but how could we then create a routine with these products? We could put it together as follows in the morning, use a cleaning product followed by a moisturizer and finally a sunscreen. at night we start with a cleaning product followed by a treatment and finally a moisturizer. As I said before, these active principles that are used to treat comedones are usually quite irritating, so their use must be progressive and our skin's tolerance to them will have to be evaluated . If tolerated, they can be combined with a night treatment but I do not recommend it from the first moment. It may also be useful to have a gentle cleanser for those days in which our skin is a little more sensitive or dry due to the use of so many acids, I recommend that you visit your dermatologist to advise you on the preparation of a correct daily routine for the treatment of closed comedones.
